Kit care, shipping and return

These terms apply to every PT kit Lake Balance sends to your lab. You agree to them when you request a PT program.

• You are responsible for the PT kit from the time you receive it until Lake Balance (or the next participant, if we direct you to ship it on) receives it.
• Pack the kit in its original packaging, following the packing instructions. Damage caused by improper packing is your responsibility.
• If you ship the kit with your own label, declare an adequate value (use the replacement value stated by Lake Balance). If the kit is lost or damaged in transit, you are responsible for filing and pursuing the claim with your carrier.
• Before shipping, check every item against the kit's contents list and instruction sheet. Report anything missing or damaged to pt@lakebalance.com before you ship.
• Please return the kit within one week of receiving it. At Lake Balance's discretion, a $295 fee may be charged for each additional week you keep the kit.
• Results are released once payment is received and the kit's return shipment shows in transit with the carrier.
